Hi Nicola and Alison,

We request a parent questionnaire for entry to Pre Kindy & Kindy (3 and 4 yr olds, I’m in WA).
We interview (with classroom visit) all families (ask general questions of interest, any developmental concerns etc) and share these notes to teachers although some more information sometimes comes with the questionnaire as it is closer to their start date!

We use Funnel and requested specific questions to be made to link to profiles then made it via a basic form.

Questions include:
– Names of previous school / day care that your child has attended and years of attendance.
– Does your child speak a language/s other than English? If yes, please comment on your child’s fluency in this language/s.
– Does your son have any additional needs or abilities of which we should be aware? Yes / No — then offer a condition if they select ‘yes’ to explain.
– Do you have any special concerns about your son’s social skills, behaviour, or wellbeing? If yes, please indicate your concerns below. Yes / No — then offer a condition if they select ‘yes’ to explain.
– Do you have any concerns for your child commencing at [insert school]?
– Is your child toilet trained? yes / no. with a help text: Please note it is a requirement for your son to be toilet trained to begin in Pre Kindergarten/ Kindergarten. additional text box with anything teachers need to know about this.
– What would you love to see your child achieve or experience during their time in the ELC
– Is there any additional information you’d like to share about your child or your family?
– family connections to [insert school]
– a check box of ‘I have uploaded any relevant reports for my child including any psychologist, diagnostic, medical reports to the Confirmation of Student Details form’
